Health Insurance
Children's provides employees an opportunity to participate in one of two types of health insurance plans: a PPO- Preferred Provider Option; or an HMO- Health Maintenance Organization. Employees share a portion of the premium costs, and premiums are deducted on a pre-tax basis from an employee's pay. Coverage types are offered for employees and employees plus their families. Coverage is available the first of the month after the date of hire.
PLAN CHOICES
- CHILDREN'S HEALTH PLAN
Children's Health Plan is a PPO (preferred provider organization) self-insured health insurance plan. Participants in the Plan can choose to go to either the preferred providers or they may choose to go to any provider they choose outside the network.
- HEALTH MAINTENANCE ORGANIZATIONS
Currently several HMO's are offered in addition to the Children's Health Plan as an option for health insurance coverage to CNMC staff and their families. The HMO's all contain dental coverage, vision coverage and prescription drug coverage.

Dental Insurance
Children's provides employees an opportunity to participate in one of two dental plans: the Children's Dental Plan (an indemnity dental plan) or the Consumer Dental Plan (a scheduled fee-for-services dental plan). Employees share a portion of the premium costs, and premiums are deducted on a pre-tax basis from an employee's pay.
Coverage types are offered for employees and employees plus their families. Coverage is available the first of the month following date of hire.

Vision Insurance
Children's offers staff the opportunity to elect to participate in the Vision Services Plan. The Plan provides comprehensive and quality eye care at a minimal out-of-pocket expense. This is a PPO (preferred provider) type program.
Employees share a portion of the premium costs, and premiums are deducted on a pre-tax basis from an employee's pay. Coverage types are offered for employees and employees plus their families. Coverage is available the first of the month following date of hire.

Retirement Plans with Employer Matching Contributions- 401(k) and 403(b)
Children's is excited to offer employees the opportunity to participate in either the CNMC Affiliates' Tax Deferred Annuity (403b) Plan or the CNMC Affiliates' 401(k) Plan by making pre-tax contributions from their gross salary.
After meeting service requirements, Children's will make a 100% matching contribution, based on the employee's contribution, up to 5% of their gross salary.
Employees can also withhold additional but unmatched money in the Plans, up to the maximum as allowed by the Internal Revenue Code.

Flexible Spending Accounts - Medical and Dependent Care
Children's offers staff the opportunity to elect to participate in flexible spending account(s) (FSA). An FSA is a benefit established under Section 125 of the Internal Revenue Code. It is designed to provide employees the opportunity to pay for certain medical and dependent care expenses on a pre-tax basis. Money deposited in your FSA is not subject to federal income and social security taxes and most state and local income taxes. In other words, the money is deposited on a pre-tax basis before taxes are withheld. As a result, this money is not treated as taxable income. The FSA, therefore, is a valuable benefit that helps you to increase your net income by reducing your federal and state income taxes. The FSA plan includes a medical spending account for tax free reimbursement of out-of-pocket medical, dental, vision, prescription expenses and a dependent care spending account which can be used for tax free reimbursement of eligible dependent care expenses.
Coverage is available the first of the month after the date of hire.

Group Term Life Insurance
Group term life insurance coverage to include accidental death and dismemberment coverage is provided to eligible staff at no cost to employees. The amount of coverage is a multiple of base salary depending on your staff position.

Supplemental Life Insurance
Children's offers you the option of purchasing additional life insurance coverage through payroll deduction. The supplemental life insurance program is available to provide additional financial protection in the event of the death of the staff member or of a covered dependent.
During new employment eligibility, this coverage is available on a guaranteed issue basis. This means that evidence of good health will not be required to purchase coverage. Since this life insurance coverage is fully portable after separation from Children's, this is an excellent opportunity to add to your existing life insurance coverage or obtain new insurance at low group rates.
The employee pays the supplemental life insurance premiums through payroll deduction.

Long Term Disability Insurance
A long-term disability plan is established to insure income protection of staff who become totally disabled for 90 days or more as a result of accident or illness, once accrued sick leave has been exhausted. The plan pays a percentage of the employee's regular monthly base salary according to staff position. The coverage is provided at no cost to eligible employees.

Educational Assistance
Children's encourages and supports our employees' on-going educational and development needs by providing an educational assistance program. After six months employment you are eligible to apply for educational assistance.
Programs eligible for educational assistance include: Undergraduate and graduate courses or programs which lead to a degree (other than the last available degree for that profession or program, such as a doctorate) and which are directly related to improving the performance of the employee in his/her current position.

Annual Leave
Eligible staff accrues vacation based on the number of hours worked, from the first day of employment at Children's. Vacation is accred per pay period, according to staff level, and years of service.
The number of weeks accrued varies from 2 weeks to 5 weeks per year.

Sick Leave
Sick leave benefits are provided to ensure income during periods when employees must be absent from work due to personal illness or injury. Eligible staff accrue sick leave up to a total of 12 days annually, based on the number of hours worked.
